Three Rivers Health District - Vaccine Update

Three Rivers Health District is collecting practice information for a Provider’s List that they will use to contact your practice as soon as they are ready to administer the vaccines. By giving your practice information, it will help expedite your prioritization in vaccine appointments. You can also specify if you would like to be on Standby status, ready to come in when they have vaccines readily available to administer day of.

Please note: Three Rivers Health District includes 10 counties of Gloucester, Mathews, Middlesex, Lancaster, King William, King and Queen, Northumberland, Richmond, Essex, Westmoreland.

 Please include the following information:

1.     Name of Dentist/Practice

2.     Location of practice

3.     Point of Contact (dentist or staff)

4.     Phone number (1 or 2 reliable numbers)

5.     Email (can be more than one)

6.     No. of employees and estimate no. of how many would want the vaccine

The VDA is in regular contact with the VDH and will keep members informed as we learn more about state and local plans.
