The Virginia Department of Health has been working diligently to ensure that the healthcare community is kept abreast of the ever-evolving COVID-19 pandemic and the challenges it has posed. 

While over 2,100 providers that have submitted their intent to provide the vaccine when it is distributed, just 103 have been completed in entirety, including the CDC Provider Agreement and provider profile.  

The Provider Intent Form cannot be saved and must be completed all at once. Further instructions can be found here. 

Once a Provider Intent is submitted, VDH will contact the applicant to move forward with finalizing the CDC Agreement and Provider Profile. VDH is working through the Provider Intents as quickly as possible, so please provide accurate contact information and please be patient.
